FSSL and Hood College Hammerheads Team Participation Waiver
By registering my child, I give my permission for my child to participate in the FSSL Hood College Hammerheads Swim Team program and agree to the following.
Information
By participating in the Frederick Summer Swim League (FSSL) Hood College Hammerheads, information about our swimmers will be exchanged between teams, some of which will end up in the public domain.
The following information, and only the following information, shall be acceptable for publication in any form, including but not limited to the World Wide Web or publicly posting at meet venues, regarding any registered swimmer without express written authorization from the swimmer or the swimmer’s parent or guardian:
● Name
● Age (in years)
● Team affiliation
● Time and place in event/heat
● Any awards or honors
Photo and Video
Since most of the League’s meets are held in public areas, it is understood that meet participants may be photographed or videotaped during the events. These images may appear in publications, websites, or other media. FSSL, and all teams within the League, have agreed to a policy which will ensure that while pictures of minors may appear on their Websites, no names will accompany such pictures. Consistent with USA Swimming, no cameras, cell phones, or other recording devices etc., are allowed in the starting area, locker rooms and bathrooms, or any areas prohibited by the host team. Drones are prohibited at meets.
Physical Health
I am fully aware that the program he/she is about to participate in involves physical endurance. Further I attest that he/she is in good physical health and he/she has no physical or health restrictions, which would inhibit him/her from participating in this program.
Liability
I do hereby and for my heirs, executors, administrators, successors, and assigns, release and forever discharge the Hood College Hammerheads Swim Team, sponsoring organizations, FSSL, their directors, administrators, managers, coaches, officials, staff, agents, and members from any and all claims, actions, expenses, and compensations whatsoever that may arise or accrue out of the above-mentioned activity. I hereby declare that I voluntarily assumes all risks inherent and further declares that I will hold the directors, administrators, managers, coaches, officials, staff, agents and members of the Hood College Hammerheads Swim Team, sponsoring organizations, and FSSL harmless for any and all injuries, whatever the extent or nature, that may arise from this activity, and during transportation to and from the activity. I hereby declare that no promise, inducement, or agreement not herein expressed, has been made to me, and that this release contains the entire agreement between the parties, and that the terms of this release are contractual and not a mere recital. I hereby declare that I am signing this release of his/her free will, without threat of coercion.
I have read and fully understand and concur with the above policy and waiver of liability. I indicate my agreement by continuing with the registration process.
